Yes, when I get really bad stomach spasms, the pain gets so bad in my back as well, that my back ends up hurting worse than my stomach. I was crying so loudly the other day from it. I've found that if I catch it early and ask my husband to rub my back, it seems to release a lot of air (I burp a lot) and it keeps the pains from getting worse. I also use a heating pad when it's doing this.
I used to be able to associate this pain with being constipated and if I could make myself go, it would help the pain ease up a bit. Lately, it's been doing it a lot since I stopped my Crohn's meds to focus on the arthritis. Librax sometimes helps during these episodes, but try having someone rub your back (I have my husband start at the bottom and work his way up) and see if that will make you get some air out. I have no idea what is happening in my body when this happens and I've told my GI about it, but he never really said what was going on. I thought it was just me who did this! But, when it happens to me, it becomes major, not just a little crampy. It becomes an "I think I might die" kind of pain.
