I got approved for Remistart in March of this year (I've been on Remicade since 2008, but didn't know anything about this program). I just got my EOB from my insurance company for my March infusion and sent it to Remistart today. Basically, you send them the original EOB for your infusion from your insurance company and they will send you a "Rebate Card" (basically, a credit card) that you can then apply toward your bill. I'm including the link for the patient information flyer and the application. They are both PDF files. Your part of the application can be completed on your computer and then printed off. Your doctor needs to complete a section before it can go to the company.

I've used Remistart as well - it's a great program!  it doesn't always cover all of your infusion costs, but every little bit helps. 

Your doctor will need to sign a form for them and fill out some information, but most doctors are familiar with the program and what needs to be done.  The folks at Remistart are always helpful too if the doctor/you have questions.

I did Remistart as well. The only thing you have to provide after the doc signs the forms are your EOBs from your insurance. Then, a few weeks later, you get the card OR you can get a check (you have to request it) if you have set up outside billing from your doc. The program lasts 1 year, or 8 infusions, then you can get into another program they offer, but that one is based on financial need I think.

I agree that the customer service there is wonderful. I got my checks quickly, and they were able to backdate me, as I didn't find the program until after my 2nd infusion. It was a real help to me!
