Biopsy results surprised me

Hi all,

Had my scope last week, which came back clean. This in itself was a surprise to me, since I have been feeling so sick lately with fevers, bloating, intermittent pain and some occasional eye irritation...not to mention crushing fatigue. The biopsy results came back with "minimal chronic inflammation" in my rectum. I was really surprised at this because my last surgery was in the ti, a long way from the rectum. I would have thought the inflammation would have been closer.

Could this explain my symptoms? Seems too minor to even count. Could the biopsies closer to the ti have missed spots of inflammation?

Having MRE friday so will know more then, I guess. Just wondering. Thanks!

Did they biopsy anything higher than the TI?

My inflammation has been far apart - proctitic and inflamed cecum first, then on my next scope proctitis and small ulcers in the TI, cecum clear. Plus gastritis on EGD.

So what's the surprise?? We have some 20 feet of small intestine and the colonoscope can only reach less than 2 feet into the TI. That leaves some 17-18 feet of small intestine unexamined.

Trust your symptoms, the disease is active.

No, the gi just went up to the anastamosis site at the very end of the terminal ikeum, so there is a lot of small bowel left to investigate with the mre.

The surprise is I don't have any rectal symptoms at all. No c, no d. Never have, even when I was at my sickest before surgery 6 years ago. So, inflammation there seemed to come outta the blue...or brown (lol sorry!)

Just wondering really if such minimal inflammation in the rectum could make me feel so crappy or if it is just a remnant of old inflammation (or new stuff just barely getting going). If my small bowel shows signs in the mre, I will know that is the culprit. I just know they will say I am fine and have ibs (like the gi did after last week's scope) and I will be left feeling frustrated and slightly insane, because that is what always happens to me. :(

In my opinion yep, I think it can make you feel crappy!

I don't think inflammation in the rectum is going to cause "fevers, bloating, intermittent pain" like you describe in the OP. I've had chronic inflammation in my rectum and when it gets bad I can feel it (more lower bowel gas, difficulty passing stool).

When I first fell ill I was having diarrhea and bloody stool and my GI diagnosed proctitis (after a year of my complaints). I treated it with local meds and got relief for some of my symptoms, but it wasn't until I started oral meds (Asacol) that I really felt better, and finally Pentasa took care of the diarrhea.

So it's not surprising that they found inflammation in the rectum. I would not assume that is causing all of your symptoms tho.

did your doc give you rectal meds?

Having CD means alot of unpredictability, it's not the easiest disease to always pinpoint and since no 2 IBDers are the same, that makes it even tougher to pinpoint. Of course you can very easily have the symptoms you are having regardless if there's only a bit of inflammation in the rectom (known as proctitis) because this disease is very individualistic. There are some patients with very little inflammation or disease activity that fall sicker than a dog and others that have alot of inflammation but still manage somewhat better than another.l

There are also the extraintestinal manifestations that can come with having IBD which some only experiance while their CD is flaring and others that are in remission will still experiance some issues with extraintestinal manifestations.

Nothing is written in stone when it comes to IBD.
