Humira or Remicade

Humira is typically given once a week or every other week by injection.  Remicade is typically given once every 2 months by infusion (IV line at an IV clinic or hospital).  In my opinion Humira is easier to take because you do it yourself so it's on your scheudule and you don't have to travel to a hospital.  But there are pros and cons to each.

One problem with anti-TNF medication is you can develop anti-anti-TNF antibodies which attack the medicine before it can help you.  Humira is a 100% human protien, so in theory it is more recogniable by your immune system and you are less likely to develop these anti-TNF antibodies.  Remicade is a "chimeric" antibody - it is part human protien part mouse protien.  The theory is that the mouse protien is more easily recognized as foreign by your body so there is a higher chance of developing an immune response.  There are techniques to lessen the odds of this happening though (co-medicating with Immuran or MTX, or pre-infusion with steroids).

The feeling of frequently needing to urinate sounds like a urinary tract infection to me.  I don't believe there are any known side effects that would cause such an urge except that Humira (and Remicade) make you more susceptable to infection.  So it sounds like there is no medical reason not to try Humira again.  Since you are more susceptable to infection, I would talk to your doctor about why he thinks you had the urinary problem before, and what steps you can take to lessen the chance of it happening agian.
