Budesonide or Prednison??

Last spring my mild UC turned to really bad Chron's (starting with ulcers in the whole mouth/throat,ascending and descending colon, fistulas, eyes damage, water in almost all joints, couldn't move for 4 weeks, and sepsis, CRP was 300). They brought me to life within 2 months with 30 mg pred, lots of antibiotiks, pentaza and azatioprin. I was out of pred in the mid of July, and azatioprin started to work in September. Unfortunetely, they had to reduce Aza from 150 mg to 100 mg, and after that in October it started to worse - little diarrhoea & bleeding, but the lab results still were fine, but gradually everything went worse and now I have a few ulcers in my mouth&throut, diarrhoea&bleeding, CRP is 55). I could not bear prednizon very well, so my GI let me to choose which medicine to take - Budesonide 9 mg or Prednison 20 mg. I am confused now - not sure whether Budesonide will help with mouth ulcers (but maybe it could be treated with some throat medicine?) and descending colon (it might help with ascending, I believe, as it next to Cecum where Budesonide is released). Also, the label says Budesonide starts taking effect in 2 weeks, which is too long, as I am getting worse every day, and memories from my last spring flare terrifies my. But I am afraid of prednizon too. What are your experience? Suggestions? Now I am on 6 g of Pentaza, 100 mg Azatiorpin, and also was prescribed antibiotics. And I have to chose Budesonide vs Prednison asap. What are your experience? Suggestions? Many thanks to everybody who will respond.

Have you considered the biologic meds like Remicade or Humira? I'm sorry you're having such a hard time. Budesonide/entocort is very efffective for the terminal ileum, but I agree - the descending colon would not be covered.

Thank you, Kazbern. Here in Lithuania it is very difficult to get the biologic meds administered, usually only young people get it, and I am 50. I am thinking what if I use budesonide foam for the terminal ileum, maybe it would help in this way? Thnaks.

What problems did you have with the Prednisone? The budesonide did not work for me at at, and all of my disease is in the terminal and distal ileum. I was surprised because I thought that's what that drug was designed for. Pred isn't a good long term solution, but maybe it will knock you out of the flare. Also the budesonide probably won't work for your throat at all. Sorry you are going through this

Sorry that you are suffering. More than one year and a half, I started 40 mg pred and I was doing perfectly well. Since I started tapering,I became worse. It was Entocort (budesonide) along with methotrexate that made me feel I'm good. I was even asking my doc about the utmost duration for taking Entocort, and he said it could be extend to years. In your case, I think you should start with pred for a quick relief , then taper it quickly and switch to Entocort. I think I missed the point of why did they reduce the dose of azathioprine. I was on 175 but I failed this med all together. Also, why don't they up pentasa to 8 tablets,4g?

gumby44: from Prednisone I'm having Cushing's syndrome, hyper hearth beat, sweating, etc. This time I've healed my throat with Tantum Verde (benzidamino hidrochloridas)

minnietoty: azatioprine was reduced because due to it my leucocytes dropped down too much, this means my immunity was knocked down), so I'm now on 100 mg azatioprine (for my weight (60kg) the full doze would be 120 mg). Thet increased Pentasa up to 6g, now reduced again till 4g. And why you did failed Azatioprin?

So, I went on Budenofalk, also Pentasa 6g, Azatioprine 100mg and antibitiocs, and in a week time my CRP from 55 till 9 :), the flare is getting better, they cut off antibitiocs, reduced Pentasa to 4g, and said to go on Budenofalk for 2 months, then cut off and see how it will be doing. I hope my flare will be getting better and better, although my leucotytes dropped down a little again, they said it might be due to Budenofalk and that constant blood control is needed, and if leucotytes is dropping down more, then Azatioprine should be reduced more too.
So, I feel better now, and will see later how I am doing in a longer run. :)

glad you are feeling a bit better. I didn't tolerate azathioprine but 6 mp worked well for me. They are pretty much the same drug but the way we tolerate/sythesise them is different.

I also find budesonide 6mg keeps my inflammtion at bay with no side effects unlike pred which affects me badly like you.

I hope to go back on 6mp and come off budesonide shortly as I now in a good remission.
