I had something very similar, possibly the same, a few years ago. My left hand and wrist swelled massively and was painful, I saw several doctors who concluded it was just my arthritis manifesting in a different way and was told my whole Crohns med regime would need to be altered to help it - BUT as my crohns was fine at the time I was reluctant to do so, and fortunately I didn't as eventually I was given an orthopaedic ultrasound on it and it showed it was actually tendinitis and tenosynovitis (the tenosynovitis is what caused the swelling) and so was nothing to do with my crohns or arthritis and the best treatment for it was a simple steroid injection into the effected area! So glad I trusted my instinct and didn't change my med regime (which at the time was methotrexate and remicade) which could have opened a huge can of worms with my crohns!
Perhaps it's worth requesting an ultrasound?
