Hi, i had an apointment with the consultant a few weeks ago because my symptoms have flared following a long remmision. I had lots of bloods tests done and i'm due a colonoscopy soon. Today i recieved a letter stating that my TPMT is normal at 102 and that i'm at low risk of azathioprine or mercaptopurine induced side effects. I have no idea what any of that means?  The consultant did'nt discuss any medication with me at all and the only meds i was ever given in the past has been pred so i'm at a bit of a loss and would be very grateful if someone could enlighten me? TPMT is an enzyme. People with with low TPMT or missing this enzyme cannot properly metabolize azathioprine or 6MP. Evidently your doctor is considering prescribing this medication to help control your CD. Azathioprine has helped maintain remission in some people for long periods of time. I know a woman in her 70s that has been taking azathioprine for almost 17 years. Prior to the azathioprine she was in and out of the hospital constantly with big small bowel bleeds from her CD.

It is a bit mystifying why the doctor didn't discuss this test with you so you would be prepared when the letter came.

British doctor, no mystery about it. Most consultants I've seen offer up information of their own accord about as willingly as Gollum giving up the ring to Frodo.

In case your consultant doesn't mention it, azathioprine takes at least 3 months to start working, so don't expect results quickly. Same applies to 6MP, or mercaptopurine, if you're put on that instead. The two meds are extremely similar, so it doesn't really make any difference which one you're put on. Good luck.

I had normal TMPT levels, but I still ended out with elevated liver enzymes after starting 6MP and had to discontinue it. Make sure that if you go on 6MP that you have follow up blood work. You are at higher risk for pancratitis or hepatitis, but it is a drug that has been very successful with lots of folks.

Thank you for your replies, they're really helpful. I must admit i'm more than a bit worried about the side effects after googling, i need to have a good talk about it the next time i see my consultant.
