Crohn's arthritis or tenosynovitis

Saw another orthopedist for a second opinion. I didn't mention that I have Crohn's. When he saw me, he said where are your tests. I asked which tests? He said rheumatoid arthritis. I said they were all negative. He said the MRI is negative but examination revealed tenosynovitis. It was then when I mentioned Crohn's. He said this is inflammation caused by Crohn's. He prescribed Celebrex, a cream and a brace.
Last week, the orthopedist my GI sent me to suggested that if pain increases, he'll perform a surgery to put a metal part to fasten the ankle.
If this is Crohn's arthritis, what can be done?

Treat it just like you would a flare up. Perhaps it is time to consider a biological like remicade.

PV, thank you very much. What do you mean by treating it like a flare up? I do take my meds regularly. I have to take celebrex, a strong non-steroidal but I'm afraid it might put me in a flare up. Both orthopedists said I need to take it to ease inflammation.
Since I have to opposing views, I'll see a third orthopedist and hen see my GI.

I see that you are on entocort and pentasa. I suggest moving on to something like remicade or humira, because obviously just the entocort and pentasa are not able to control all the crohn's symptoms.

I do not recommend taking celebrex, which is an NSAID and can make crohn's (digestive side of things) worse. But I think you should move to a more potent class of drugs to control the crohn's - by trying something like remicade. That's my opinon.

PV, thank you. Honestly speaking, I feel I'm not yet ready for biologics. Will wait for the third orthopedist's opinion, undergo some tests and see my GI for a re-evaluation of meds.

A rheumatologist might be able to help. My Crohns colitis has been in remission since bowel/bladder fistula surgery more than ten years ago but the rheumatologist has been very helpful with the other auto immune related issues like scleritis and arthritis.
about celebrex, my GI doc says NO NO NO no NSAIDS ever.

My experience with NSAIDs and enteropathic arthritis is that my GIs warn against it. That hasn't kept other doctors from wanting to prescribe it though, since I have arthritis as a main complaint. Some of us can tolerate certain NSAIDs w/o flaring, and I'm one who can take small doses of ibuprofen, but anything in the range of controlling arthritis will cause me to bleed intestinally.

For me, prednisone would only help at the beginning doses; years of Entocort and 6MP did nothing for it. Mobic, an NSAID my GP prescribed, did nothing for it either, but thankfully didn't make things worse. The only thing that ever gave me remission was my colon surgery, but that only last 1.5 years.

Remission of the disease is about the only thing that's really going to make a difference. With my insurance here in the US, biologics are cost prohibitive.

I would advise calling your GI and getting his advice on whether you should go with the Celebrex treatment since it can cause the CD to flare, especially since you've been having so much trouble. -Joy

I'm already flaring up. Pain in the ileum is excruciating. Took all possible antispasmodics but in vain. There is d also but I don't think it's a problem. But I'm very weak.
I think the swelling I have in my hip could be related to this tenosynovitis thing. I did not tell any of the orthopedists about it. I'll try to see my GI and tell him about all problems.
