rectal pain with BM

Hi all. It's been a long time since I've been on now. I had a follow up colonoscopy in April to see how things have been going. I was originally diagnosed 5 yrs before with a mild case of Crohns. My biggest issue has always been rectal flareups. (proctitis flares) and one of the issues I deal with is pain with BMs. I use lidocaine and hydrocortisone to keep things under control. Anyhow, I was told with this last colonoscopy there was no sign of inflammation in the rectal area and things seem to be in remission however, I am currently dealing with pain with BMs again and it's been bad. I can't entirely tell if I am passing harder BMs or if its just the skin (inflammation) causing the pain but past few days have been hellish. If things look great as my gi dr says than what is going on? Anyone wind up going to a procto? Any ideas as to diet possibly making things worse? Literally the only recent change for me has been eating regular ice cream as opposed to frozen yogurt. Not sure if the milk fat would be an issue or what. Any thoughts or opinions would be appreciated. Thanks.

Thanks, kazbern. I don't think this is the issue as I can actually get pain just having water hit that area while in the shower. It feels like several little cuts causing a lot of big pain. Yet, the whole thing begins with a BM that feels like a struggle to pass even though I don't think stools are particularly hard when this happens. GI mentioned seeing a procto which I am not relishing the thought of. I feel he could diagnose if it's a fissure himself. Don't know why the need for more drs.
