I don't know if my experience is of any relevance to yours whatsoever, but in the year or so before my first surgery I had a small fissure just outside my butthole - so not actually in the anus itself. It was agony every time I went to the toilet, even if I passed 2 drops of liquid stool, and would continue to hurt for up to an hour afterwards. Anyway, the only thing which made it better was going on (yet more) steroids. Never really figured that one out - whether it had anything to do with the action of the steroids themselves or whether fewer BMs gave the fissure time to heal. The fissure would inevitably come back as I tapered off the steroids and relapsed, but at least I got a break from it.
I have no idea if steroids would do anything for you or not. It's the only thing I can think of to try :-/ Oh, and it may be worth considering a second surgical opinion.
