Faecal Calprotectin High but All Scoop Normal

Hi,

So as my post before this, I'm the one that already had endoscopied, colonoscopied, swallowed pillcam, all came back clear. CRP blood test also negative.
The only one positive and high only the Calprotectin test at 573. Doctor baffled by this but based on all the scoops, he declared I have IBS-D

Last May I infected by amoeba in hospital (talk about unlucky), hospitalized a week with metronidazole. Call it miracle descend, after 4 years suffering loose stools, from June to early this August I became perfecrly normal, with good appettite and well formed stools, free from any symptoms. Until....

BAM 9 days ago it started hurt again, stools became like porridge then oatmeal like then just flat watery pile with foams. And my stomach constantly hurts a lot to the point I can't sleep or waking me up at night. All medicine prescribed for IBS-D like spasmomen, Librax and anti-diarrheal I have taken, which usually a bit help and the symptomps subsided in 4 to 5 days now nothing works! Up until I wrote this my stools still like watery oatmeal and foamy, even if I had took anti-D .....

Is it really IBS??? They said IBS never lasted more than a few days......im really scared if it's like the first diagnosis, which is Crohn......help!

Hmm, this is a tricky one.

First, there are sometimes false positives with fecal calprotectin. It's a fairly reliable indicator of inflammation somewhere, but it's not perfect. It's also known that it can rise substantially when there is an infection, and it can stay elevated for quite a while afterwards.

Have you had it re-tested recently? Was it still really high, or was that 573 result from a few months ago or last year?

Honestly, to me this sounds like something more than IBS is going on, but I don't think it has to be Crohn's either.

Were they able to take biopsies of your colon? What about biopsies of the end of your small intestine - could they get those during the colonoscopy? If so, what did the biopsy reports say?

Have you been tested for celiac?

Have they tested you stool to see if there is undigested fat or protein in it?

Have they checked your gall bladder and pancreas to see if you are producing enough bile and pancreatic enzymes to digest things properly?

Have you been tested for food allergies, or have you tried elimination diets?

Yes my doctor said that too, he said it's false positive since I've been endoscopied and colonoscopied twice, last year on March and this year on May. Both showed really nothing inside.

The calprotectin test also done twice, last January and last March, first it's 370 then 573...

Everything on the report said everything normal....

I have been on diet,changes nothing. I don't have any allergies. If it's IBS they said they will notice some change. Moreover I always has loose stools, if it's IBS it can sometimes be constipated right? I never once constipated.

For gallbladder and pancreas they have performed ultrasound and also found nothing.
I haven't been tested for celiac or stool for undigested fat etc. What does those fat and protein in stools implied?

Oh and since I have this illness I have this condition in my tounge which resembles geographic tounge, they will cleared up ,become normal again, in sync with my period, then after period it will formed bit by bit in a month until next period. I'm really scared since they said Crohn can have that condition. It's inflammed tounge they said......

If it was crohns it has a distinct pattern which the gi would have seen. With normal biopsy's, I don't think you need to worry about having IBD going on. I think you posted this in this in IBS forum & a mod responded to you. Some folks can have either diarrhea or constipation, or both at various times.

Doctor didn't biopsied anything from the colon, since the walls of the colons are normal. At least that's what my doctor said. I'm worried because it's been my 12 days of loose stools (like watery oatmeals) and whatever medicine I take seems taking no effect. Half down of my spine and legs been aching too since...I have seen my doctor again yesterday, but he also frustrated and looks like don't know what to do anymore with me since all medicine he has tried on me. Does it really IBS? Then why it lasted so long, they said if it's diarrhea for weeks it goes more toward crohn....

The doctor should have taken biopsies even if no inflammation was visible with the scope. There are plenty of conditions that have normal appearance on the scope but abnormal microscopic appearance in the biopsies (two forms of microscopic colitis, plus other immune-related forms of colitis).

Ask for the blood tests for celiac disease and for other autoimmune diseases and immune deficiencies.

Ask for stool tests for fat malabsorption.

Have you tried a short course of steroids like prednisone? That could be worth a shot for a couple of weeks.

Has done blood test for autoimmune diseases, also came fine. Moreover IBD won't showed up at this kind bloodtest doctor said.
Stool test for digestion also done, found nothing, no protein or fat or anything.

Before when I was diagnosed with Crohn doctor prescribed me Salofalk for a month 3 times a day. No improvement either.

Don't know what to do anymore.
