A perforation is a hole. The bowel wall is quite thick and made up of several layers; a perforation would go through the entire wall, leaving a hole for the intestinal contents to leak through and cause infection, peritonitis and sepsis if left untreated. A cut is more superficial and may cause profuse bleeding if it's deep enough, but it's not the same thing as a perforation.
I mean, technically a fissure is a cut; I wonder if a fissure was what the GI was referring to. At any rate if she thought you had an intestinal perforation, MB, she would have admitted you into hospital. I think you're doing the right thing. You could also be anaemic from blood loss, so it's worth getting your iron levels checked out as well.
