How far do extrintestinal manifestations affect treatment?

Sorry for posting a lot these days but I'm sick and struggling with current meds: Rifaximin, probiotic, Pentasa and Entocort. I have EN but not too much. Only two small ones on the right leg and another small one on the right foot. I know that this is EN because of the pain and I have experienced this a lot in he past. My GI has told me before that whenever I get new ones, I have to see a dermatologist to examine them. He saw fading ones before and knew that they are EN. I read that the examination entails taking a part of the EN and make stitched if the wound is big to send it for a lab. Is this true? Suppose it turned out that it is EN, would it make a difference in diagnosis or treatment? My GI is finally convinced after struggling with him for years that I have one form of IBD, not confirmed yet. I feel that these tiny ENs I got are trivial and am embarrassed to see a dermatologist. I used in the past to have plenty on both legs, face, arms and back and never complained.
Has any one experienced change of treatment coz of EN?

Thank you, Kazbern. They are just very small. How is he going to make an incisional or excisional biopsy? I read they can be 0.4 inches big. Ok, I'll go but would there be any benefit in treatment?

Pb4, thank you very much for these pieces of advice of dealing with these nasty nodules. Well, my GI wants the dermatologist to confirm for him that this is Crohn's. So far, I have no naked eye ulcers. Biopsies showed only chronic proctitis and mild active ileocolitis and all my symptoms conform to IBD. My GI has seen them himself. I too don't think will like him to cut any of them . But doubt that he might convince me , as has always been the case with procedures I hated, to confirm diagnosis.
