Stelara - Who's on it and What Dosage?

Hello, I'm considering attempting to get Stelara off-label. I know it will be an uphill battle but I have failed all anti-TNF's. But I also know that plenty of people have been successful at getting insurance to pay for it off-label. My major question for those people is what dosage and frequency are you taking Stelara? Additionally, what was your loading dose? My GI isn't exactly sure what to prescribe since the clinical trials are testing multiple dosages. Thank you for any information you can provide.
