Well, to me it sounds like you have a stricture- this is a narrowing of the space through which watse travels. It's caused by recurrent inflammation. Your disease is severe and a stricture can be a real pain. The stricture wasn't so bad that the doctor couldn't get past it, but it was apparent. Also seems like the doc had some trouble identifying normal landmarks (parts of the intestines which appear generally the same in everyone and are used to determine where the scope is inside of you), most probably because the inflammation is so much that those normal landmarks are either in the wrong spot, or not discernable at all.
All of this makes me think that infliximab at your current dose just isn't cutting it anymore. You may need to increase how often you get the drug, or switch to a different drug.
I had severe Crohn's-Colitis with stricturing and fistulae. Mine got so out of control (drugs stopped working at all) that I had to have a colostomy installed. All of this is caused by out of control inflammation, you need more meds.
Note: I am not a doctor, I just play one on stage.
