Hi Katie, and I'm sorry no one has answered your post.
I was diagnosed from my biopsies and the GI who did that scope actually said from looking at my colon he felt sure it wasn't Crohn's, though I had multiple patches of ulcerations. He said my ileum "looked good" via the scope, but the biopsies showed microscopic ulcerations, so yes, it can look okay to the eye and still show up in biopsies.
The first GI I went to thought I had UC. The second GI, the one I mentioned above, diagnosed Crohn's. The last GI I saw tried to change it back to UC even though I already had a fistula, saying I had a "backwash" in the ileum. The surgeon he referred me to who did my hemicolectomy said it was Crohn's and told me he had never seen fistulae with UC though I have no idea if that's always the case.
