RV fistula pain

I am now 35 1/2 weeks pregnant - almost there! Over the last month or so, my recto-vaginal fistula has been kind of causing some pain. I'll occasionally get a twinge sometimes, but the last month that has increased a bit. I asked my GI about it, and he said it was pretty normal, especially if there is scar tissue being pulled as I get bigger and maybe dilate a bit. However, yesterday and today, the pain has shot up a lot. I get sharp stabbing pains on both the anal and vaginal sides, and they will last for a few minutes. It happens several times a day, and it will hit like a 7 on my pain scale. I'm not sure what it means, and I'm not sure which specialist to call. I have an appt with my OB on Friday so I might just wait it out until then. I don't think there's an active problem with it, but the pain gets so intense that I almost can't walk. Any thoughts?

Thanks! I'm not sure if it's quite what's going on. I know it is specifically my fistula - it's the exact same spot that hurt so bad after the plug surgery and sometimes after intercourse if the angle is wrong. It feels like there is a sharp stick being stabbed in that same spot. Unless this affects the fistula indirectly?
