I have 3 setons for fistulas and a malecot drain for a perianal Abscess. My setons aren't the cutting kind, they are basically there to keep the tunnel open for the fistula to drain and heal. I started Remicade back in April for this and a mild Crohns flare that showed in my colonoscopy. As for cutting setons, I've heard that you go in periodically and they are tightened and cut slowly to help repair/heal the fistula tracts. The other kind of seton just stays put without needing any special attention til the Dr is ready to remove it. My setons don't really bother me anymore. I swim, bathe, and use the bathroom normally. Just keep it as clean as possible. Goodluck, I know it's tough. I've had my equipment, as I call it, in since December.
