Crohns and Celiac???

Can I assume your celiac diagnosis came from an upper endoscopy with biopsy of the duodenum?

When you say a repeat EGD does not show celiac but shows lymphocytic colitis, do you mean you had an upper endoscopy and a colonoscopy? What exactly was found in the colonoscopy biopsies? Do you have a copy of that report?

Being positive for ASCA IgG and IgA signals to me only a small concern for Crohn's. By themselves, those two markers aren't highly specific for Crohn's disease. The blood test done by Prometheus labs uses the two ASCAs plus two other antibodies to bacteria, plus a couple of inflammatory markers, plus a couple of genetic mutations, plus a couple of other autoantibodies - and with all that, it's still only maybe 90% accurate for determining whether or not a person has Crohn's.

They can't diagnose lymphocytic colitis from the colonoscopy itself - they have to diagnose it from the biopsies taken during the colonoscopy, and that report usually comes in several days later.

The second opinion tomorrow will probably base their opinion off of the biopsy/pathology report, if they have access to it. If they have access, ask for a copy for yourself and tell us what it says.

If they don't have the biopsy report, contact the office of the doctor who did the colonoscopy and request a copy. It should tell in detail what the pathologist saw, and that should tell whether or not you have lymphocytic colitis.
