How often for a scope-based on my parameters

Hey looking for an opinion. Why? Because I have lost faith in my GI deciding on scope frequency and treatment due to $ versus need. I have been told you don't need one except every 10 yrs,3 yrs, 5yrs, 3yrs, 2yrs, every year...every visit the answer changed until recently...last 4 years insists I need one every year. Nothing has change since 1990.
I had resection 1990, 1 fistula 2001, taking only mesalamine, limited ER visits unless I eat food I should not, narrowing at surgical site has not changed since post resection 1990, no new findings on any scope since resection. I don't even see my GI every year. 1 major flare up in last three years that lasted a week with no treatment other than ensure only for a few days.
How often for a scope based on this?

Lamb...wishing you better...I always say I am one of the very lucky crohnnies. Pretty much post resection, I have been very well. I would say 3 flareups that required steroids and the fistula were my biggest issues in 29 years. I just don't see the need to get a scope every year when the report never changes and neither does my care...guess I'm bitter too that when i do bring up any issues, they get brushed under the carpet.

You could ask/try to have regular bloodwork (including crp) and regular stool tests for fecal calprotectin, and base the need for scopes on those things along with symptoms.

For example, if you go a year with no symptom changes AND no changes in fecal calpro, then no colonoscopy needed.

If your fecal calpro starts trending up and/or symptoms start worsening, consider colonoscopy.

I agree with Beave. I think yearly blood tests with inflammation markers and fecal calprotectin seems reasonable and then you can stretch the colonoscopy to every 3-5 yrs if you have no new symptoms. My CRP never elevates so we stopped testing but for me the sed rate does elevate. When I have been in flares in the past I become anemic which tells us that I have bleeding somewhere. I've been in Crohn's remission since my surgery in 2014, but have dealt with lots of infections and the last bowel obstruction from eating carrots.
