What have biopsies shown? Sometimes they are key to differentiating between UC and Crohn's.
location of disease activity is a factor in distinguishing the two, but it's not the only thing that matters.
But in some cases, it's just really hard to tell which it is. I think something like 10-20% of cases of colitis are considered "indeterminate," meaning they can't tell whether its UC or Crohn's. And sometimes even with a definitive UC diagnosis, the presentation has some features indicative of Crohn's. It's not always black and white.
