It sounds to me like you might unfortunately be somewhat paying the price for taking this disease too lightly. Even once treated, you may have been undermedicated. Imurel, (Aza) is a very common immunosuppresent, but may not have been enough to control your symptoms at that point.
You do sound like a candidate at this point for an anti-TNF. Remicade would be the first I would consider, and is still considered the state of the art treatment for Crohn's. Remicade has shown to be extremely effective in healing fistulae as well.
Can't really comment on the procedures you've had/having with the setons, but I'm sure others with experience will chime in.
As I mentioned to another recent poster experiencing despair. You can and will get this disease under control. It will take the right treatment plan. Stay positive and be very clear with your GI about advancing your treatment. Take a family member with you as an advocate. Keep us posted. Things WILL get better.
