diabetes and blood sugar and the resulting effects.
Blood sugar spiking to 187 is not normal. From what you wrote about
the GFR result and eyesight, I'm guessing that you have diabetes and either need medication or a really strict eating plan to keep your blood sugar at normal levels. Normal GFR is 90 and higher. Read this:

Of course there are other factors that can affect kidney health but diabetes is one of them and this is why we need to keep our blood sugar as near to normal as possible. I am puzzled that a nephrologist would tell you that everything is fine based on your numbers. 70 is not a normal GFR result.
Have you seen an ophthalmologist and had a thorough eye exam? This is not about
glasses. There are tests for glaucoma, cataracts, and retinopathy that only an ophthalmologist can do. High blood sugar can cause all of these conditions and there are medications (eye drops and procedures) that are prescribed to get these under control. If I were you, I would make an appointment with one of these specialists asap.
I don't know about
the other conditions you describe. I do know that you absolutely need to get your blood sugar down to normal. You can work on this yourself and might not need medication. Please see your general doctor for this.
I don't know about
regulating the blood pressure but I do know that uncontrolled blood pressure can also damage kidneys (as well as heart). Do you take blood pressure medication? There are classes of blood pressure meds that also protect the kidneys. ACE inhibitors are one kind of blood pressure medications that do this. I take one of them.
I wonder about
the herbs you are taking. They are not usually regulated by any health agency and some of them can affect our health in negative ways. Do you think it's possible that the herbs are causing palpitations or irregularities with blood pressure?
Do you have a good general doctor that can order a full range of blood tests and possibly refer you to specialists if necessary? I think you need to start with some conventional lab tests. If you write what you eat every day for meals, I can help you understand what is best for your blood sugar. Do you test your blood sugar every day?
