I sometimes suffer from rls and I DO suffer both day and night and sometimes my arms seem involved. Sometimes it will start in the late afternoon and I can stop it by walking. Several things tend to bring it on, such as anxiety, caffeine, exhaustion, cold medicines, stress and lack of exercise. If I get too little sleep, I find that not only does my pain skyrocket, so does the possibility that I will suffer from rls the next night. The exhaustion and the cold medicine are tend to be my worst episodes.
I don't know if your name is because you are anxious, but if that is the case - could maybe the doctor treat the anxiety and see if that helps with the rls?
My sister suffers from it, as well. She tried one of the medications and she said it made her feel like she was in a fog. She gave it up after two days, though she said that it stopped the rls.
I hope you find something that helps you. I know I feel very lucky that I figured out what brings it on for me.
