Are you seeing a Urologist? If so, you might consider a Uro-Gynecologist or a Urologist who specializes in women. I have a long history of urinary issues, and I finally found a female Urologist who specializes in the treatment of women. I just felt like her approach was more comprehensive and suited to female issues.
Also, the itching would make me wonder if there is something else going on than just a straight up UTI. Have you changed anything lately - soaps, detergent, feminine products, etc? And, of course, the antibiotics can cause problems with yeast, giving you a double whammy.
As for OTC remedies, some gynos (mine included) are going back to using boric acid suppositories for stubborn yeast infections. You can get them online or make your own. Just make sure to use pharmacy grade boric acid (You can get it at Walmart). You can buy gelatin capsules and fill them yourself. Oh, and while boric acid is believed to be safe as a vaginal suppository, it can be toxic if ingested orally.
