I think that I would go down in person as if I had never contacted them. Not mention anything about your history. It is not the receptionists job to know your medical issues. If you feel you need to justify why you need an appt, say you are having extreme pain that has been going on too long and you think it needs to be assessed by a dr. (after trying everything that you can think of to relieve the pain at home.)
Get the appt. When you arrive, then fill out the paperwork or take the papers home to bring back on the day of the appt.
When I call my dr. for an appt, I explain that I need an appt to speak with him about some trouble with my meds or I have a need for a sick appt. I don't give her the details. (and I like her very much. :) I just don't discuss my trouble with anyone there except the nurse that day and the dr seeing me.)
If you like him, I would then tell him about the battle you had to endure to get an appt to see him. :)
