My father in law moved from a MELD score of 37 to 41 in a week and next day it when to 42. His bilirrubine and INR are way up and this is causing the high MELD score. He is now hospitalized but we don't know what to expect. Have you seen this before? since the MELD range is from 6 to 40, we don't understand what this new score outside the range means. Any information you can share will be appreciated. Thank you!

Adding to previous message, I should say my father in law has Hepatitis C and has know about it for the last 10 years. Thanks.

Hello, Mlicool, and welcome to the forum.  I am sorry that your FIL is so sick.  Is he on a transplant list?  If so, he is certainly at the top of it, if he has been through evaluation and found to be a good candidate for transplant.  If not, you can expect the worst.  He is in liver failure and will not live much longer without a transplant.  Most transplants occur with a MELD in the mid to high 30s up to low 40s.

I assume your father has never had treatment for hep C or it didn't work for him.  At this stage, his only chance of living is a transplant.  I have been through evaluation myself and was placed on the waiting list July 18th of this year.  My current MELD is a 22, so I feel transplant is not going to happen for me anytime soon.

Where is your FIL hospitalized?  Is it a transplant center, as well?

My husbands meld score was 15 Aug 13 and by Sept 15 it was all the way to 39 and then it lingered at 38 ...til Sept 26 .....I too was worried about it going over 40 also ...I ask doc what happens if it does ..he said the patient get gravely ill but they still do transplant people but they won't live very long without transplant. Doc said they just transplanted someone with a meld of 42 ..and they were doing well ...my husband just had his transplant this Tuesday ....and is doing good but I honestly don't know how many more days he had left without transplant.....my husband was also hospitalized during all this which is good because the liver team follows his levels everyday updating meld score .....is you father in law at hospital that will do transplant at ? Did he do transplant work up ? What state are you in ? Sorry for so many question
