Lower right quadrent pain ONLY symptom of IBS??

My daughter who is 18 has Crohn's Disease.  Yesterday she had a colonoscopy with the hopes of dilating a stricture she had in her Terminal Ileum.  Well, when they got to the spot that is causing her constant pain - to the point she is on a liquid diet- they found no stricture and a normal ileum.  She has been on Remicade since April, and in Jan had a double dose with no relief.  She was feeling fine from about August-mid Dec. when her sypmtoms returned.

Now her GI is wondering if it IBS.  I don't buy this, as I personally have the IBS-C version.  She has no problems with D or C, just constant pain in this specific area.  He is putting her back on Azathioprine and raised the dosage of Amitryptiline she was on for miagraine headaces, thinking this might help with the pain.

What do you fellow IBS'ers think.  Is it possible to have IBS with LRQ pain the ONLY symptom?

Yes, pain alone can be a symptom of IBS, and odd as it seems to me, plenty of people assure me that you can have both IBD and IBS. IBS is what happens when you have problems, but no signs of inflammation.

Is it possible that she's actually got inflammation somewhere else, but the pain is radiating to this healthy place? I know from personal experience that pain likes to radiate.

I do hope someone's looked at her appendix?

I would ask for a abdominal and pelvic CT scan with contrast. Sometimes appendix will flare and then subside. Mine did for almost 3 years with no symptoms except occassional pain. Definetly worth checking out.

She is going for an MRI on Feb 25/08 which was scheduled since early January.  Her sypmtoms started to return mid Dec and have gotten worse.  The colonoscopy was with one days notice, but we'd been trying for almost 2 wks to get a hold of the GI's nurse to let them know what was going on.

I am wondering if maybe there is a narrowing higher up than what the other tests she's had over the last 2 yrs showed.  Maybe it not within range of the scope.  I guess that is what the MRI will show.

Her appendix has been looked at as the source many times, but her blood work always has come back fine. ALWAYS!   The ER people think that every thing is fine then.

I've been starting to wonder if she may have a trigger point in her abdomen that is causing this pain, or if Reflexology or abdominal massage is something to look at.  I  am just drawing cards here, out of desperation.

My blood work was fine also during the time my appendix was flaring up. That was why it was never discovered till they did exploratory surgery and then found it was all twisted up. They didn't have CT scans then. I believe a colonscopy goes all the way to the appendix so it wouldn't miss something in the colon unless it was a sigmoidoscopy and that only goes a small ways. If the MRI doesn't show anything as they show things entirely different then get the CT scan. I am really surprised that wasn't the first thing done especially before an MRI.

It's a tricky area, that lower right quadrant. It's not necessarily GI pain - could be referred pain from pretty much anywhere in the lower abdomen! Hopefully that MRI will reveal something that can be treated and if not they might try the anti-spasmodic approach, which has definitely worked for many people (they are good, safe drugs). The normal blood work is a good sign, but pain never is, so hopefully they can elucidate the cause soon...let us know what happens!

I think its very possible to have pain in one specific area. I dont have chronic C or D either, sometimes I will get pain on LRQ all day and then it will be gone and then sometimes i will get it on LLQ. There were times where I had back pain for weeks and thought it was my kidney.
Since the colonscopy was clear and blood work was fine, IBS shouldnt be discarded. If its something serious and life threatining, the pain should be accompanied with fever, no?. And they should have seen signs in the blood count if there is any infection. Our colon can be so confusing at times that we think it could be something else. For me ever since I upped my intake of fiber alot of the pains went away. It took awhile but it worked.

You said she has chrons disease, was she officially diagnosed by a GI? maybe that is what causing the pain?. MRI and Colonoscopy and blood work are very important and thorough test, if she does have something other than IBS they should see it in one of them.

oh yes, she was officially diagonised by a GI after colonoscopy with samples taken [can't think of the right word right now]. She definetly has Crohn's. In her TI and large colon.

She has totally defied all logic when it comes to her health and how thing "normally" present themselves. She has had many ear infections etc as a young child with no fever. She is one special kid! ;)

No, pain and fever don't go together all the time. Lots of pain without fever. Usually fever with infection and that should show on blood work but sometimes it doesn't. I still think a CT scan is called for.

well, like I mentioned, at least I think I did, she is having an MRI on Feb 25, GI said it was better as no radiation is involved. I think you can get a better look at everything inside that way too.

Just wondering if a small bowel follow through would show this?  She's had a number of these.

Yeah probably.

However,barium enema migth be better because you can compare if the cecum "fall" when you stand straigth on the plate.

Speaking for me,when they took the x-rays standing straigth,the cecum "fall".

Let me know if you find a surgeon willing to fix it in the natural position,i have a hard time finding a surgeon for this in America.

If she has no IBS symptom,it could comes from the appendice i guess.

Looking at the x-rays help to locate the cecum to see where the pain comes from.

As far as IBS is concern,pain in the cecum is usually related to trapped gas.

A lot of my health problems seemed to have started in my LRQ. They checked my appendix, ovaries (the right one at one point did have a large cyst, but the pain never went away when it did) They did a lap to check for endometriosis, they did a colonoscopy and he even got up into my small intestines a bit and nothing. No one could find anything wrong. The pain has gotten better, but is still there, and sometimes flares up pretty bad. It feels like a knife stabbing me. It's always the same spot, never moves. Doesn't get better or worse with a BM or passing gas. Doesn't get better or worse with my period, rest or moving. I do kneed real deep with my knuckle and sometimes it does help. I too wonder if it's a trigger point.

Hi,

I have had severe RLQ pain now for 4 years following a gi infection with Klebsiella. It never goes away either. I have had all the tests now except a laparoscopy. I am so frustrated. I am never hungry and feel foods sort of digests and just pushes through and does not come out well 90% of the time. The best bm I have are if I eat just straight papaya all day. I really dont know what I digest. I have tried almost every therapy, type of diet and supplements there are. I am thin, hardly can sleep from this - what feels like a blockage. I have been to Mayo clinic twice, nothing shows and they say post infectious IBS, this is ridiculous, I am wasting away. I dont believe IBS cant kill you, I am severly underweight and have osteopenia badly at 30 yrs old. I suffer headaches all the time, from toxicity I am sure. I have tried all the fibres and they just irritate but they seem to reduce the headaches, which confirms I am toxic even on a light clean diet. If I take a tbs of psyliium, I am so full I can hardly eat anything. I am on a virtually smoothie diet already and do not believe ensure and those type of drinks are healthy at all. I have taken all the antioxidents, fasted, went all raw, all cooked to no improvement. Is anyone else out there having any success with this lower right quadrant problem? I also did energy treatments for the ileocecal valve and tried to make sure it is functioning...

This is what my doctor says I have and for the longest time the LRQ pain was severe and my only symptom.  Seeing that your daughter has Crohn's though and knowing that is the familiar pain associated with it I'd bet it's her Crohn's aggravating her.  Hope she feels better soon either way though.

A lot of these symptoms can be caused by Lyme Disease. Not sure if this would be the case but worth looking into. If so find an LLMD. You can youtube the movie, UNDER OUR SKIN to watch the trailor. A lot of these diagnoses are symptoms of Lyme.
