Sounds like an anal fissure to me too.
And yes, you can have that pain without blood but if you don't address the situation there will be blood sooner or later.
I am just now getting through the same thing. It happened to me almost exactly as you described.
I did the precription ointment. (nifedipine) Seemed like it was helping at first because the pain diminished quite a bit right away. But, after two weeks of the prescribed ointment it began to bleed and the pain intensified.
The GREAT news:
Go to the drug store. Get a tube of zinc oxide. After every BM clean down there well (wipe with either wet toilet paper or medicated wipes.) Squeeaze a pea sized amount of zinc oxide onto your finger and apply to anus. (I could feel where the fissure was in my case...perhaps you can feel yours as well) Do this after each BM.
By the end of the first day there was significant change. I am 3 days into this and now there is no pain whatsoever...even after a large BM has passed. You will be amazed...and quite happy.
I can't claim credit. I found this remedy for anal fissures as i desperately surfed many websites looking for answers. (and what a wild array of answers I found!) Simple, plain old zinc oxide ointment is the ONLY way to go. Before I finally tried it, I was certain I would have to have surgery for the fissure. Here I am, three days into using it and I feel completely normal. I will continue to use it for awhile to make sure it is indeed completely healed.
Oh BTW, this fissure started AFTER I had a nasty perianal abcess. I was able to defeat that SOB without it turning into a fistula (something I was deathly afraid would happen...I didn't want to go through any of what I had read so many poor souls go through) I think some of my unconventional methods for healing the abcess brought on the fissure.
