I have had IBS with constipation and bloating for years. Now I have been diagnosed with SIBO - Small intestinal bacterial overgrowth syndrome, or Small Bowel Bacterial Overgrowth Syndrome (SBBOS).
My breath test was positive as below.

I was placed on Xifaxin 200 mg three times a day for 14 days with great improvement in mys symptoms, especially for the first few days, but total relapse occurred the day after stopping the medication.
I have been restarted on Xifaxin 200 tid for another two weeks, which again is helping, but it does not sound like it is going to be curative. And I can't afford to be on it long term as it costs $200 for a two week supply. Does anyone know what to do next?

From what I have read, it's very hard to get rid of bacterial overgrowth and you do need to take antiobitics for a while. No, it's not a lifetime thing, but I have seen, for certain types of bad bacteria, recommendations of up to 6 months (that's pretty extreme, if you think about
it!). But that's for like antibiotic-resistant c. difficile, so you shouldn't need anything that extreme.
Are you also taking probiotics? By taking them during and after the antibiotic treatment, you can help get your gut repopulated with good bacteria. Your colon is a lot like a street with houses on it: bacteria live in the houses. Now, like any neighborhood, there are only some many lots where you can build houses. Once the neighboorhood is full up, bacteria have to pass on down the road (and out of your system). Antibiotics evicts everybody from their houses en mass. If you can introduce probiotics to your system while you have a lot of houses sitting vacant, you can rebuild your intestinal neighbohood with mostly good neighbors. If you don't help reintroduce them, though, any bad bacteria that survived the antibiotics can very quickly multiply and take residence back up in all of those empty houses.
A number of doctors have found that people recover better when they do a 1-2 punch with the antibiotics and then the probiotics.

Thanks for your reply. I think I am in for a long siege. I thought antibiotics killed the probiotics too. How can one take them around the antibiotics? There doesn't seen to be a good time to do it. Why not just start them the last day of the probiotics? Does anyone do that?

If you take the probiotics while you are on the antibiotics, it tends to lessen the severity of the diarrhea that most people get while on antibiotics. That's pretty much the only reason why people take them both together. Yes, the antibiotics will kill off the newly-introduced good bacteria, but they can at least work a little and help you out for the short time they're in there. I have to take probiotics while on antibiotics to try and stem that diarrhea problem (works pretty well, too). Because probiotics give me terrible gas, I always take my dose right before bed so that I'll sleep through the worst of it.

Just make sure to take your probiotics at lease 2 hours after you're done taking any antibiotics for the day.

It's a good advice about
taking probiotics, as antibiotics along with pathogenic bacteria kill normal intestinal flora. Xifaxan is a very good antibiotic for SIBO, as it works only in intestinal tract, and is not absorbed into your blood stream. However, if you hardly can afford it you can talk to you doctor about
other antibiotics. Antibiotics that are used for SIBO include fluoroquinolones (ciprofloxacin, norfloxacin), tetracycline, amoxicillin with clavulanic acid, second generation cephalosporins.

How is SIBO diagnosed? What are the symptoms?
I have gas, bloating and incomplete evacuation and thought it was IBS, however my doctor feels that in the absence of constipation/diarrhea and/or abdominal pain it is unlikely to be IBS. H. Pylori has been ruled out and I am scheduled for colonoscopy. Does anyone else identify with these symptoms? I am anxious to put and end to this problem. I seem to get better when there is less stress and when I exercise a lot and watch my diet, however when busy it is difficult to stay away from stress and keep up with the exercise and diet. Please help!

I also have SIBO and had taken 500mg for 2wks and then 200 for another 2wks. I have been eating Activia yogurt everyday prior to taking the pills. I feel so much better than i did. Have to say Still have Distention alot. But i have had it for 9 months before diognoised. Is is bad to keep taking Xifaxin for along time? Im uncomfortable being on the medicine for so long. I have to say my stomach grew to 37 1/2 inches around and being on the medicine it went to 35. So I felt it was working. But as of today its back to 37 so im not sure if i should repeat again the medicine. Is it true once you have it, that it never goes away?

forgot to mention that i was waiting for people to come up to me and ask me when i was due because i look 4 month pregnant. Very frustrating!

I also have IBS and found what works for me is Bran Bud cereal put 2 tablespoons of it in your cereal every morning and I have to say i go everyday now. Its like 5 dollars a box but last along time. You only need 2 tablespoons of it and its not a pill or laxative to make you go. Gets you regulated!
