Has your friend been tested for Celiac and IgA deficiency? I only ask because I am very thin and have been misdiagnosed with IBS for the last 7 years before finally being diagnosed with Celiac. Eating a gluten free and low-FODMAP diet helps me a lot. A lot of people with Celiac are very thin, although some tend to be overweight instead.
Even if your friend does not have Celiac, she should still look into her diet. She should see a dietitian and do a diet trial to see if a low-FODMAP diet would ease her symptoms.
However, as something she can do by herself, she should try going lactose free for a month and see if it helps. If she still experiences symptoms, she should then try going on a low-fructose diet for a month. Then see a dietitian if she's still unwell.
