Going to see Gastro, is there a list of questions?

I'm finally going to see the gastro.  They sent me this "book" of forms to fill out before I go and I thought in the same spirit of things, I would compile a list of questions to take with me for them.

Is there a list of questions available as a resource here?  If not, since this is my first time to see a gastro, what do I ask?

I've been suffering for years with IBS-C and just in the past 6 or so months, everything has gone haywire in my stomach and intestines. I'm so tired of taking tests that I don't need or that show things as normal.  There is definitely something wrong...

I want to be informed and want to inform the doctor - I know there is a good and bad way to do this.  I want to get on and stay on the right foot with him.

Here are a list of questions the book states you should ask for finding the right doctor: Do you have patients with IBS?   (You want the doc to say yes.) What do you think causes IBS?   (You want the doc to say s/he's not positive...blah blah blah... it's complicated... there are many triggers.) How do you diagnose IBS?   (You want the doc to mention the Rome III Criteria and that eliminiating other conditions with similar symptoms are a must.) What role does diet play in IBS?   (You want the doc to be aware of what you eat can cause triggers - as well as food allergies and intolerances.) How do you treat IBS?   (You want the doc to say there are a variety of options - diet, exercise, stress reduction, maybe dietary supplements.   Doc should not focus only on medications, they don't work for eveyone and they're not a cure.) OK, those are the most important.  Go armed and don't be shy.
