Malar Rash or not?

I am still in the process of getting a diagnosis for whatever is going on with me. At this point I am having bad joint pain in fingers, knees and feet/ankles. I also have a worsening of my acne or rash on my face and wanted to see if you guys could peek and offer an opinion? The rash is on my cheeks and across my nose - I also have a bit on the top of my eyebrows, between them? My nose appears to have acne like bumps, but the rashy stuff on my cheeks isn't raised like pimples.

I've been bounced around from doctor to specialist for over a year with no diagnosis, and see a new rheumy in October due to high ANA again (1:640 homogenous). Thank you for any input!
