It could easily be that the doc, as part of the overall treatment plan, wants you to know "this is what we'll do now, then we'll change it up in three months to keep the little buggers off balance." Not unusual to see a note like that at the end of MD instructions.
I agree with the previous comments about adding in the abx one at a time. This is good because if you have a nasty response you'll know which one likely is the culprit.
the doxy/amoxi/plaquenil combo should go OK, but I think you should stay alert when you add in the Tindamax, you might need extra fluids, lots of detox baths, etc.
It's actually exciting to hear your doc is willing to get after it like that! High hopes for you!
