I would start a thread "Looking for LLMD Near Long Island." Make sure you have your email address showing to members under Edit Profile. We have some members in the New York area. Maybe they can advise you regarding LLMD's in your area. You can email back and forth with questions if you have any. You can check RateMDs and Yelp.
Do you have the name and email address of your local Lyme support leader? I would get that person's contact info and email him or her. Ask for feedback on doctors. You can even go to meetings and ask members for their opinions. I would talk to as many peopke as possible to get a sense of the doctor before you go.

If they never resolved, it means you are still fighting an infection. Is the neck stiffness new? Most ILADS trained LLMD's will treat until the symptoms resolve plus a few months.
Part of my problem in figuring out Lyme was that symptoms would come and go and new ones would show up sometimes. It made it hard to pin down. Now that I have read a lot, I see more and more symptoms that I attribute to Lyme and Co. Lyme and co can cause so many health issues. I did not realize that when I first thought about
Lyme and co.
