I think Peteza is right that it's not good to burn bridges; however, lyme often puts us in abnormal and difficult circumstances that healthy people don't get put into. If the only way to get to Germany in order to due a treatment is to burn a bridge, I think you probably should do it. I'm not sure how much you've looked into the treatments in Germany. They may require more than 2 weeks. If so, you may need to tell your new employer that you had a prior vacation scheduled, and you can't cancel the flight and need to take 2 weeks before you start your new job. That would give you a full month, if necessary. I think it's quite common to ask for 2 weeks before you start a new job, and I think it may be necessary if you're going to Germany for treatment.... I'm assuming this is hyperthermia treatment?
Disability is very difficult to get. At least in my state of Massachusetts. I applied for disability, and they didn't even respond for 6 months. I doubt it would be a quick and easy process.
My opinion is that your health always comes first, and your job comes second. If you're not healthy, then you can't do your job. Simple as that. I'd take the opportunity to try to get healthy with the Germany treatment.
