mrdude-
I would increase the Nystatin to the max dose (although increase slowly, but steadily). The Nystatin is very safe to take daily - it generally doesn't cross the mucosal barrier of the GI so it doesn't have to be processed through the liver, etc. (unless you have significant GI damage).
It's very important to hit the y/f overgrowth fast and hard - as hard as you can tolerate. The due-off sx can be quite intense, sometimes rivaling your overgrowth sx. So it's important to ramp up to max doses - even higher as quickly as can be tolerated.
Also, the regular manufactured Nystatin pills are full of fillers and not enough Nystatin. PURE Nystatin powder is much more effective. You should be on min 1 million units per day. More about this in that Y/F post in the "New to Lyme?" thread. I was on 2million units per day of the pure powder that was very effective.
Most MDs won't prescribe at a beneficial dose so sometimes you have to engage a "work-around" - ramp up slowly to higher doses at the beginning of the month and then end the month at double the dose - almost evens out. Most people need to stay at the higher dose for another month so that's when you have to get creative...
Local vs systemic antifungals
People are often a cautious about taking antifungals Rx, which is advisable for all drugs and herbs. However, it's important to understand the differences between the local vs systemic antifungals.
As described, local antifungals like Nystatin generally don't have to be processed through the liver and can be used more freely. Systemics like Diflucan are processed through the liver and enzymes need to be checked monthly.
There is much more info in the thread I referenced so might be a good idea to check it out to determine how you should treat.
