ILADS guidelines

Can any one point me to the ILADS treatment guidelines? I'd like to present these to my Doctor when I follow up with him this week. I likely have acute lyme disease and do not want it to go chronic. Initially I was only prescribed 3 weeks of Doxycycline. I relapsed after I finished my treatment. He's since put me on another 30 days of doxy (2x100 mg/day) but I feel that won't be long enough given that I went undiagnosed for 1-1.5 months and was off doxy for 2 weeks during the relapse.

I'm slowly responding to the doxy again (been back on it for 5 days) although not as fast as I would like.

I've read through the article on their website but it is quite lengthy. I was hoping there was something quick I could show my Doctor. My GP is pretty good but he always seems rushed even if there are very few other patients waiting. I get maybe 5 minutes to say what I have to say and anything beyond that he shuts down pretty quickly. Typical of our health care system in Canada.
