My partner has has issues like these that were relieved by chiropractic care, craniosacral therapy, and breathing and relaxation techniques. The nerves and muscles in the area were inflamed and in spasm, so they produced soreness and a feeling like choking. It probably doesn't feel like it, but it could be structural.
Do you have a pulse oximeter ($25 online) to make sure you're getting enough oxygen? That can help put me at ease somewhat when I experience air hunger or throat issues.
My experience has been similar to yours in some ways - herxing the whole time on treatment, underweight, and not having fewer symptoms after treatment. For me, I think part of the puzzle is having mast cell activation syndrome. It took me a long time to figure it out because I don't have skin issues, which most people seem to have from it. After switching to a low-histamine diet and taking quercetin, treatment and supplements don't make me react much, if at all. DNRS also has been helpful for me.
Are you interested in trying other forms of treatment? Or just experiencing treatment fatigue at this point?
The sentence about feeling like the world has thrown you away really got to me. I have had that feeling on many, many days. The level of demoralization that this illness brings is something few people can comprehend without going through it.
