In my very limited personal experience its not always the same thing, but perhaps I'm splitting hairs.
In the DC area specifically, the state boards are still attacking the licenses of LLMDs and so many great doctors are in hiding or simply not calling themselves LLMDs. LLMDs have also started referring patients to open minded non-Lyme specialists in order to treat patients and you wont find them on any LLMD list either.
My last three LLMDs have all had their licenses revoked both in Maryland and Virginia so the climate is getting pretty terrible here. This might be why my current doc (an inflammation disease expert) refuses the title. Same treatment, same protocols, different title.

I thought that was all in the past.
Dr J moved his practice a few times (was sued more than once and even went bankrupt) - and in Washington DC he hasn’t been hassled.
Is it because DC isn’t a State? Different rules ? I’m Canadian so not familiar
