not exactly - several organs in the body are special and sensitive in that they cannot be directly exposed to everything that is in the circulating blood supply
the body therefore has a semi-permeable membrane between the general blood supply - and the fluid inside those organs - to allow only the subset of the things in the blood, that the organs needs to survive to get through - and keep the damaging things out - in the case of the central nervous system - this is called the Blood Brain Barrier(BBB). it must allow oxygen and hormones etc in - but not pathogens, larger food molecules, antigens etc.
if its function is impaired that's exactly what will start to happen - and the results can be unpredictable.
inflammation is always going to be a feature and this is likely immune mediated ( driven by the immune system).
so that's where things start to get complex - as each persons immune system and immune reactions are different.
pathogens may play a part of course as they can and do make their way into our brains ( human brains of even healthy persons are not 100% sterile ), but other things are known to dysrupt the BBB - infections, toxins, chemicals, some pharma drugs - even some herbs to some extent.
so its not that the BBB itself courses damage - but that things getting to the brain because it fails to keep them out - can initiate a cascade of inflammatory events - most likely driven by the immune system - and it is these that can damage the brain
ref COVID - COVID is known to have adverse effects on endothelial cells - which are the type of cells that make up the BBB - so its certainly possible that COVID could have some impact on the BBB - although i haven't seem any direct evidence of that - but from memory i think your issues predate COVID by some years - so i would not assume that to be a large factor in your case.
this is of course only one possible mechanism by which altered brain structure may have occurred - my main point being that if you can test for and find antibodies to neuronal structures ( commercial tests exist for this) it could give you some direction for treatment options.
