What should I ask Dr.?

I am going for my next visit this week.  I am so frustrated.  I feel horrible.  Still making it to work everyday but anxiety is through the roof, heart beating through my chest, joint aches and pains, now pins and needles down one leg, it goes on and on and on.  I am on 500 mg amoxicillin 3X a day with probenecid.  I was feeling better after 4 months on this but then he stopped to do doxy for erlichiosis and everything came back.  I just don't feel like I am being aggressive enough.  It took me 4 years of hard work to get diagnosed and now 9 months into treatment, I just feel I need to take a more agressive approach.  Any advice?  Thanks.
