Sorry you have had trouble with your health. From the sound of it you simply have not been treated adequately for your Lyme infection or infections. The give away clue was your saying that you were still fatigued after the first treatment.
Successful treatment comes from treating two months beyond the cessation of all symptoms. Have a look at our "New to Lyme?" thread. There is a lot of info and good resources there. In particular have a look at Dr Burrascano's Treatment guidelines and visit the ILADS site (International Lyme and Associated Diseases).
While you NP sounds very caring, you really need to be treated by someone who has experience treating Lyme thoroughly and effectively. I would strongly encourage you locate a Lyme Literate doctor for treatment.
