I think it varies and depends on what type of treatment is required. I know some of the babesia meds are really expensive as are IV's. Several thousand per month in some cases without insurance.

Some doctors are willing to substitute alternatives if needed or use less expensive treatments in a combination such as herbs, dietary changes etc.

There's really no way to calculate ahead of time what it's going to cost, unfortunately, although, yeah, IV meds are VERY expensive!

My insurance recently decided to stop paying for my antibiotics because of the whole IDSA/ILADS thing - so my doctor suggested going to walmart or costco. So far costco has been working out pretty well for me - might be an option for you if you have one nearby.

Also if your doctor wants you on certain supplements - sometimes you can find good deals online, especially when you buy in bulk. I know there are also a lot of coupons floating around out there - I remember once I just did an internet search and ended up with $20 off my lunesta prescription - I don't know if they do that for antibiotics as well, but there are lots of little shortcuts out there.
