I agree with Traveler. You should do everything you can to make sure this is going to be the right choice before you start to get involved with this doctor.
I was upfront over the phone. I told the person that I believed I had Lyme and that I wanted to make an appointment to be tested and treated for Lyme and co-infections. I think it would be difficult for a LLMD to hide the fact that they are treating Lyme within their own practice, so I wouldn't worry about
discussing it with the receptionist or the nurse.
I was also upfront with the doctor. I had already been through the wringer with several other doctors who were looking into everything from Lymphoma to RA and Lupus. All of those tests had come back okay. They were testing me for everything BUT Lyme and I had strong evidence to support a Lyme diagnosis. I had virtually all the symptoms of Lyme and several co-infections and they all started after several known tick bites and a very high fever and rash. I had no health problems prior to that. So I wasn't going to beat around the bush with another doctor. I wanted to find one who would at least agree to test me via Igenex for Lyme.
In your situation, you already have a positive test? I would mention that on the phone. You pretty much already have a diagnosis and just need someone to treat you appropriately.
I did extensive research on the doctor before I went there to make sure I knew that this LLMD was on the ILADS side of things. By talking to the people who offered the list, you should be able to tell what type of approach this doctor takes.
I would ask over the phone what type of labs he/she uses for testing (only because if they use Igenex, they are more likely to be Lyme-friendly) and what treatment approaches are used before you even make the appointment to make sure it lines up with what you want.
