Rouleaux just means your red blood cells are stacking up in columns when visualized under a microscope. This happens from a number of different causes, including improperly drawn samples, coagulation abnormalities, inflammation, etc.
In other words, by itself, it is very hard to say what is causing this.
Lyme/coinfections may cause this, but there are many other possible causes.
