State Disability - What does one put?

Hi,

I've got a State Disability Form to fill in and am not sure what to put. I'm worried that if I put Lyme Disease alone then it will get dismissed on the basis as the CDC has labeled it as only a temporary condition.

Does anybody have any experience with this?

I also have Babesia, Bartonella, EBV and a high viral load although the doctor is not sure what viruses other than the EBV to attribute this to. Should I put everything down including some of symptoms like chronic fatigue or arthritis etc?

Any advise would be greatly appreciated

We may have to do it for our daughter. I don't look forward to it at all and I'm concerned it will make her feel more depressed, feeling that she's a burden to us, which she's not.

I spoke with a disability attorney, and she said they have had success getting disability for someone by calling it "tick-bourne illness". This is in a state that doesn't officially recognize Lyme disease. Of course, we will also list the pertinent symptoms, and more recognizable conditions.
