Where can I send a tick for testing?

I would like to send the tick I pulled off of my son yesterday for testing. It's also dead so does that matter for testing purposes..? Does anyone know where or how I could send it in to get tested?

I printed out the tick test requisition form from iGenex and will be sending this little bugger in. I have an option to test for lyme, babesiosis, ehrlichiosis, bartonella, rickettsia and relapsing fever. Each test is $68 each. I'm trying to figure out which ones to check off. All of them? Or only a few? AHH I don't know, so many options and my lyme brain is horrible today.

Oh and I ordered Dr. Jernigan's book and also the Buhner book and Dr. Horowitz book (along with my sauna) so I hope amazon delivers those soon!!!
