Is she detoxing? If so, she needs to do more, if not, she needs to start.
200 mgs of Doxy is pretty low, but the LLMD should address that when he sees her on Friday.
The neck pain sure sounds like the meningitis type of symptoms that Lyme can produce. There's really not much that I have found that alleviates that, although heat and possibly gentle massage may be of a little benefit.
Heat may help her hip joints, but be sure to try putting the heat on her low back, down by her hips too if directly on the hip doesn't seem to help.
Don't forget sometimes it's inflammation that can cause the pain, so ice may be better. You'll have to try both to see what works.
And please don't beat yourself up!! The thing is, Lyme is everywhere and she could have gotten it all on her own from a mosquito bite. At this point there's just no way to know - and beating yourself up will only add stress to your body and mind, which will give the infections the upper hand if it goes too far. So please, allow yourself to stop blaming you for something that you literally not only had no control over, but are likely not responsible for!!
