Please send her my sympathy - those are rough symptoms. Of course, I want to know if she's on an anti fungal...
But also, I think yes, should could be dehydrated. And I think the abx cause insane changes in the microbiome of the gut, which can also cause the same elsewhere and vaginal yeast/fungal overgrowth AND an overgrowth of other bacteria, because of the imbalances created.
Either y/f and/or bacteria could cause stuff to "grow out" of the vaginal area and the genital skin on the outside is pretty sensitive to begin with, but a growth can cause agitate and cause extreme discomfort.
I would first definitely have her drink more. But also, have her try applying some organic coconut oil (which is an anti fungal and also an antimicrobial) - not much, just a little to see if there is either a reaction (burning, stinging, agitation), which would indicate a likely y/f and/or bacterial overgrowth, but with repeated application it could also resolve the issue and could eventually be soothing and healing. Or it could maker her feel better right off the bat.
I've even read about women who fill ice cube trays w/ coconut oil and insert them like a suppository (particularly after sex) to help soothe and heal and prevent overgrowth.
Hopefully this will tide her over until she can get to her gyno. She needs to be evaluated so that she can be placed on the appropriate treatment so it doesn't get worse or cause her additional stress.
